All players must register with the Arizona High School Hockey Association(AHSHA) and the Brophy Hockey Club to participate. To register for AHSHA click on the link below:
AHSHA Registration
Brophy Registration – To register for the Brophy Hockey Club players must submit the forms/documents listed below. Please double check the forms to confirm that all parent/guardian and player signatures are complete. Mail completed forms to:

Skaters will not be allowed to travel with the team or get on the ice for practice or a game without a complete set of forms and identification documents.
1. Consent to Treat and Medical History - This document is mandatory and required by AHSHA. AHSHA requires this form be turned in but not the medical history if you prefer not to complete that section.

2. USA Hockey Code of Conduct - This document is mandatory and required by AHSHA.

3. USA Hockey Registration Confirmation - You received this when youregistered at www.usahockeyregistration.com. If you have misplaced this paper, go to www.USAHockeyregistration.com and request a duplicate copy. AHSHA needs a copy of the receipt with the bar code so it can be scanned. All players (and coaches) must have a current USA Hockey Registration form turned in.
4. School ID – A copy of your Brophy ID card must be submitted.
5. A Copy of Your Birth Certificate - The league will need to verify your date of birth for USA Hockey. Passports or other official documentation will work. If you played last season or during the summer, we have a copy of your birth certificate already. All new players for the 2011-2012 Fall season must provide a copy of the player’s birth certificate or other official documentation showing date of birth and place of birth.
6. Brophy Photograph Release - Brophy requires that we provide a release for any photographs taken of the students/players.

7. Brophy Permission to Travel Form – Brophy requires that we have permission slip forms filled out in order to attend events like the pre-season training camp at Manresa.

8. Brophy Hockey Club Code of Conduct – This document must be read and signed by both players and parents. It sets forth the Club’s policies, rules, and expectations.

9. Notice and Release Form – Must be signed and returned before any player will be allowed on the ice.

Club fees for the 2011/2012 season will be $1600.00/player. A $500.00 deposit will be due by 8/10/11.
Payment Options:
One Payment due 8/10/11 - $1,600
Three Payments: $500 Due 8/10/11; $550 Due 9/15/11 and $550 Due 12/1/11

Club fees include 2-ice slots per week, coaching salary & Manresa club retreat fees. Varsity team players may be assessed additional fees for tournament expenses.
